If you’re on this page of the site, it means you have successfully completed the courses in the servicing track. Congratulations! You’re now ready for The Next Step - forwarding your Personal Data form to Wells Fargo Home Mortgage.

It’s simple.

Step 1. Open the PDF file of the Personal Data Form. You will need Adobe Acrobat Reader to use this form. You can type your answers directly into the document.

Step 2. After entering your answers, print the PDF file and fax or mail it to Mortgage Career Paths. Fax it to (202) 721-0187, or mail it to:

If you have Adobe Standard 6.0 or higher, you can enter your answers, save it to your computer, and email it with Mortgage Career Paths in the subject line to campusmbaeducation@mortgagebankers.org. Name the personal data file with your last name (example: jones_mcp.pdf).

Step 3. You will receive confirmation of receipt of your Personal Data Form via email. Your form will be forwarded to Wells Fargo Home Mortgage for their review. We will be unable to respond to telephone or email requests about the status of Well Fargo’s efforts to schedule discussions with you. However, if you have not heard from a Wells Fargo representative within four (4) weeks of receiving your confirmation, please send an email to the address above.
